If you can't create a new world with the same seed and try again it should be there most of the time bit again they don't always spawn. A week before 1.4.4 I had a random seed where the sword shrine was on the surface, and relatively close to spawn for a large world.The Terragrim is a 1/50 drop from the Sword in Stone, you can't get a seed for it.
